We respect your privacy and take the confidentiality of your personal health information seriously. You have the right to request a copy of your health care records at any time. To protect your information and ensure proper identity verification, we do not offer an online records request form. Instead, all requests must be made through direct contact.
 

How to request records:

• Call our office directly to request records by phone (Contact Dominque ​or Dr. Ashley Ward-Hines) see below or

• Submit a written request by email to Dr.AshWard@gmail.com
   

When submitting a request:
Please include your full name, date of birth, current contact information, and a detailed description of the records you are requesting. Additional verification may be required prior to release. This direct contact process allows us to safeguard your data, confirm identity, and minimize unnecessary electronic storage of sensitive information. If you have questions about the records request process, don't hesitate to get in touch with our office, and we will be happy to assist you.​​​
